Health transformation extends across a connected sequence: creating clusters, establishing Health Holding Company, transferring responsibilities, defining populations, developing capabilities, changing funding, and strengthening cluster independence.

Each step creates the foundation for the next.

A cluster first needs to understand the facilities and teams within its area. It then needs to understand the population it serves and its health needs. The cluster can then build financial and operating systems, define performance indicators, and develop its relationship with funding and regulatory entities.

This process turns a group of facilities into an accountable institution.

Institution building requires governance, leadership, data, financial systems, purchasing capability, performance management, and a culture focused on outcomes. It also requires continuous clarity of authority among the Ministry, Health Holding, health clusters, and funding entities.

Initiative launches create early movement. Institutional independence and operating efficiency create lasting transformation.

Transformation therefore needs managed stages. Every cluster advances according to readiness, with clear criteria for movement from one stage to the next. Measurement focuses on the cluster's ability to manage population, resources, journeys, and outcomes.

Vision 2030 provided the direction. Institution building converts that direction into a daily operating model.

The target is a network of clusters that own decisions, capabilities, and accountability. These clusters operate as accountable care organizations inside one national system with shared objectives.
