Each health cluster carries responsibility for the population of a defined area. A citizen's address connects that person to primary care centers, hospitals, and specialist services within the cluster serving that area.
This connection gives the cluster a clear picture of population size, needs, and service utilization. It also creates direct responsibility for access, quality, and continuity.
Primary care becomes the closest gateway. Assessment, referral, and follow-up start there. Specialist services operate within an integrated care network. Cases requiring additional capacity move to another cluster or a private provider through a clear operating and funding pathway.
Geography becomes an instrument of accountability. The cluster knows whom it serves. The funder knows which organization carries responsibility. The citizen knows where the journey begins.
This design raises the importance of regional capacity distribution. A specialty gap appears through patient transfers and referral costs. Long waits appear through demand moving to other providers. Limited primary care capacity appears through greater pressure on emergency departments and hospitals.
